Intake Process

EMPOWER offers a largely self-serve, user friendly, intake process. The organization’s website contains all new patient paperwork available for download.  The new patient intake packet, legal forms and rating scales provide valuable information to EMPOWER providers which help inform next steps, in customizing a best practice assessment algorithm. The collection of screening tools is comprehensive, to ensure that no symptom domains are overlooked. Findings from the standardized assessment are then used to determine the level intensity and nature of services recommended. Evidence-based treatment pathways derived from the best and most current scientific research available in the field are followed by all center providers. In addition, outcomes and treatment progress are systematically tracked, in a scientifically rigorous manner for all EMPOWER patients.
